If you are leaving the hospital and require extra help at home, including home care or other aftercare, equipment, or supplies, case managers are available on each floor in charge of discharge planning.

If you are an outpatient and need extra help at home, your doctor or nurse will arrange such care with Memorial Sloan-Kettering's Case Management office.

Depending on the details of your individual insurance coverage and your clinical situation, certain services, equipment, and supplies may or may not be covered. If a service is not covered, your case manager can provide you with information about fee-for-service companies.
